Assessments provide an in-depth understanding of each client’s strengths, challenges, and treatment needs. Every evaluation includes a comprehensive bio-psycho-social review, standardized screening tools, and individualized recommendations to guide care or inform next steps.

Substance Use Assessments

These evaluations follow the ASI-Lite (Addiction Severity Index – Lite) framework and include completion and interpretation of the SASSI-4 (Substance Abuse Subtle Screening Inventory).
Clients receive a clinical diagnosis (if applicable), treatment recommendations, and—when requested—a Statement of Safety to Practice suitable for employment, licensing, or professional monitoring programs.

ADHD Assessments

ADHD assessments help clarify patterns of attention, focus, organization, and executive functioning that may affect daily life.
Each evaluation includes validated screening tools, a structured clinical interview, and individualized feedback with recommendations.

  • Standardized questionnaires (ASRS, WURS-25, DBDRS, DERS, MDQ)
  • Structured diagnostic interview exploring developmental and current patterns

Availability
ADHD evaluation appointments are offered through secure telehealth on a limited monthly basis to ensure thorough, individualized attention.
When appropriate, clients may be referred for medical or psychological follow-up for diagnostic confirmation or medication evaluation.
